  2. Providing the agency with the name, age, and grade level of each child being homeschooled

  4. Keeping records of the child's progress, including attendance records and samples of the child's work
  5. Administering an annual standardized achievement test or alternative assessment to the child, or having the child's progress evaluated by a qualified third party
